When submitting application, please ensure you have included the following:

  • (1) Completed New Camper Registration forms and questionnaire
    Or
  • (1a) completed Returning Camper registrations forms and questionnaire if your camper attended in 2009
  • And (2a) Completed health record (by a doctor)
  • And 3a) Completed school questionnaire (by a teacher or education assistant who knows your child well)
  • Most recent psychometric/psychoeducation assessment if available
  • Two (2) recent photographs

Please note: to hold a placement, it is advisable to submit the completed New or Returning Camper Registration form as soon as possible. A placement will be held for 2 weeks if you need more time to complete health and school records. Placement cannot be confirmed until we have received all the requested information.
