The forms are "fillable" pdf's. This means you can save them to your computer then complete them on your computer before printing them. You can also save them to your computer, print them and complete by hand if you prefer.

When submitting application, please ensure you have included the following:

• (1) Completed New Camper Registration forms and questionnaire
  Or
• (1a) completed Returning Camper registrations forms and questionnaire if your camper attended in 2009
• And (2a) Completed health record (by a doctor)
• And 3a) Completed school questionnaire (by a teacher or education assistant who knows your child well)
• Most recent psychometric/psychoeducation assessment if available
• Two (2) recent photographs

Please note: to hold a placement, it is advisable to submit the completed New or Returning Camper Registration form as soon as possible. A placement will be held for 2 weeks if you need more time to complete health and school records. Placement cannot be confirmed until we have received all the requested information.
